Congratulations on being selected for a HRV Booster Scholarship!
We want to be sure you get your award! It is mandatory that recipients of an HRV High School Booster Club Scholarship meet the following requirements to receive the award. Please complete the following steps and we’ll get your money on its way!
Scholarship Recipient Checklist:
Submit a transcript indicating successful completion of one (1) full semester at a technical college, 2- year junior/community college or 4-year college/university. Most students send this to us after they have completed their first term.
Submit a class schedule for the upcoming semester (typically spring term) at a technical college, 2- year junior/community college or 4-year college/university.
You have two (2) years to meet the requirements.
Checks will be made payable to the secondary education institution with a memo stating the name of the student and their student ID #. We must be provided with accurate information to ensure the recipient's school account is correctly credited.
Use this Scholarship Recipient Funds Request form to formally request the funds and provide the details and documents (listed above) that we need to create the check on your behalf.
